Grantee Forms
Project Directors for GHC funded programs must use the following documents to administer their grant:
1. W-9: Upon notification of your GHC grant award, you must submit a W-9 form by mail or fax. (PDF format, 36KB)
2. GHC Logo: Use the GHC Logo in all publicity materials related to your GHC funded program. (TIF format, 164KB)
3. Amended Budget:FOR PUBLIC PROGRAM AND TEACHER ENRICHMENT GRANTEES ONLY. If your GHC public program or teacher enrichment grant award is less than the amount requested, you must submit an amended budget form by fax or mail (and receive approval of your amended budget) prior to requesting dispersement of funds. 4. Financial Report Form: After submitting your w-9 (and having your amended budget approved, if applicable) request up to 90% of your grant funds at anytime during your grant period by submitting a financial report form by mail or fax. Requests generally take a week to process. Checks usually arrive 7-10 days later. (PDF Format, 147KB)
5. At the completion of your project, submit the following forms no later than 90 days after the end of your grant period to close out your grant and receive the last 10% of your grant funds:
